Please complete this questionnaire to request a change in wetland indicator status rating for a plant species listed on the National Wetland Plant List (NWPL). A new questionnaire will need to be completed for each individual species and each requested change in wetland rating (a single questionnaire can be used to request the same rating change for a given species in multiple regions).
